1. Proof of Age and Identification

Acceptable Documents:

- Lease or mortgage statement
- Utility bill in your name
- Rent receipt
- School registration or voter registration
- Employment letter indicating your service area
3. Proof of No Insurance
Acceptable Document: Your most recent tax return showing that you do not have health insurance.
4. Proof of Income Level
Acceptable Documents:
- All sources of household income (wages, pensions, social security, disability benefits, child support, alimony, unemployment benefits, workers’ compensation, rental income, etc.)
- For wages: Two most recent, consecutive pay stubs.
- For Social Security disability: The Notice of Award document.
