Eligibility Requirements: Step 1

Age: 18+ years of age

Insurance: You cannot have health insurance to use our services.
Residency: You must live or work in Beaufort County.
Income Verification:

If you have a job: Bring the last two pay stubs from the past 45 days.
If you live with someone who helps you: That person needs to fill out and sign an In Exchange Form* before your appointment.
If you do odd jobs: Get a Self-Declaration of Income Form* signed by one of your employers before your appointment.
If you are homeless: Complete a No Income Form* and get it signed by someone outside your situation before your appointment.
If you are self-employed: Bring last year’s tax return or a 1099 form.

*Please stop by the clinic during operating hours to pick up any applicable forms.

Address Verification:

The address on your photo ID must match the address on your application. If it doesn’t match, bring a bill with your name and address.

Social Security Number:

If you do not have a social security number, you must go to the SC Department of Health and Human Services, 1905 Duke Street to obtain a current form 3300. This is important as Welvista will require this document for anyone without a valid Social Security number.
